SELF PAY

You will be contacted prior to your surgery with an estimated procedure cost for your surgery.  The discounted balance will be due in full on date of surgery.  You will be asked to complete a financial agreement.  Any remaining balance will be due within 90 days from your date of service. 

SELF PAY – COSMETIC SURGERY  - ELECTIVE SURGERY

Payment in full must be received 10 days prior to surgery.

 
If your insurance company is not listed, it may be considered to be part of one of the networks listed above.  Please call our office at 860-247-5555 and ask to talk with the billing department for more information.
